New PhilHealth accountable forms from banks
PhilHealth Regional Office VI has gathered representatives of the banking industry to discuss anew the PhilHealth collection of accredited collecting banks (ACBs).
Both government and commercial banks are tapped to do collection of premiums of the national health insurance program. PhilHealth introduces certain accountable forms utilized in the transaction.
The existing Contribution Payment Return (CPR) forms (ME-5, MI-5, and Min-5) shall be replaced by the unified accountable payment receipt called PhilHealth Agent’s Receipt (PAR). PARs shall be issued by Accredited Collecting Agents (ACAs) to acknowledge premium payments from paying members and employers. Paying members shall also be required to accomplish the PhilHealth Premium Payment Slip (PPPS) for the payment transaction.
As procedure, Payor accomplishes the PPPS and supply information ranging from PhilHealth Employer Number (PEN) / PhilHealth Identification Number
(PIN) / PhilHealth Organized Group Number (POGN) to Payor’s name (complete Member / Business / Agency’s name), membership type (voluntary / OWP / Sponsor / Private / Government), applicable period of premium contribution, and, the amount to be paid.
Upon accomplishment, the PPPS as well as the amount to be tendered is handed in to the teller / cashier. Member then has to secure the validated copies of PAR (original white copy as its file; yellow for the employer to be attached to the EMPLOYER’S REMITTANCE REPORT (RF-1). In the case of Individually Paying Member, the yellow copy shall be submitted directly to PhilHealth.
On the other hand, the bank’s teller validates the PAR using the information stated in the PPPS. The copy of which are distributed, original (white) and 4th copy (yellow) shall be returned to the paying member while the (pink) remains as file copy of the ACA and green) shall be attached to ACA’s ABSTRACT OF DAILY COLLECTION (RF-2).
As to the readiness of these forms, PhilHealth assures delivery of the forms timely for its implementation effective July of 2010. (Larry Tabsing)
